Biography

As the Director of Internal Medicine for the New York Presbyterian -Ambulatory Care Network of Primary Care Practices at Columbia University, Dr Fleck has experience in designing and implementing system change in urban primary care settings to improve chronic disease, specifically in diabetes and tobacco cessation. She has worked closely with the New York Presbyterian Leadership team in attaining Patient Centered Medical Home accreditation.
